- In the Partner Portal, navigate to Websites and then select the website with the care plan that you want to change:

- Select the Care Plan tab at the top of the page and then click on Change Care Plan in the top right:

- Select the new care plan in the pop-up modal:

- Confirm your changes by clicking OK:

Note that we will process the care plan change on the first of the upcoming month.
